SSA 44 Instructions and Required Documentation
Form SSA 44 is the official way to request a reduction in your Medicare income related monthly adjustment amount. The form asks about your life event, your income, and your current situation. Getting it right matters.
Here is what you typically need.
Your most recent tax return or proof of current income. A letter from your employer if you retired or stopped working. A death certificate if your spouse passed away. A marriage certificate or divorce decree if your marital status changed. Documentation of any other life event you are claiming.
The form itself asks for your tax filing status, your adjusted gross income, and any tax exempt interest. You also estimate your current year income. Social Security compares the old number to the new number and decides if a reduction is warranted.
One common error is using the wrong tax year. Social Security calculates IRMAA from the return filed two years ago, not the most recent one. The SSA 44 asks you to identify that specific year. If you pick the wrong one, the form does not do its job.
Another error is leaving off the tax exempt interest. That number counts toward the IRMAA calculation, even though it is not part of your adjusted gross income. Miss it and Social Security may reject the form or ask you to resubmit.
A third error is not signing and dating the form. That sounds obvious, but it happens, and it delays the review by weeks.
You can do this yourself. The form is public and the instructions are available on ssa.gov. But if you want it done in one pass with a checklist of exactly what to attach, a prepared packet saves time and lowers the risk of a denial.
