Form SSA 44 is the official form you submit to Social Security to request a reduction or removal of IRMAA when you have had a qualifying life event. It is not a tax form. It lets you explain why your current income is lower than the old tax return Social Security used.

Only certain life events count for lowering IRMAA. They include marriage, divorce or annulment, death of your spouse, work stoppage, work reduction, loss of income producing property, loss of pension income, and an employer settlement payment. If your situation is not on Social Security's list, a packet probably will not help.
